It's time again to participate in the annual Westside Business Leaders
Association/Wal-Mart Shopping Spree!
All monies earned from this event will support Black Creek District
Events. For each donation of $1.00 this district receives $.75 cents.
Two winners will be selected. Winners will receive a gift
certificate good at any Wal-Mart Store or Sam's Club in Jacksonville.
PRIZE DRAWING WILL BE ON SATURDAY, December 1, 2007.
You DO NOT need to be present! Winners will be notified by
phone.
Prizes are as follows;
First Prize $1,000.00
Second Prize $ 500.00
Each ticket booklet contains TEN (10) - perforated tickets.
Tickets are to be given for a $1.00 donation each. For each sale,
have the purchaser complete the stub portion with Name, Address, & Phone
# -YOU RETAIN THE STUB for turn in along with the money!
The purchaser receives the ticket portion which entitles them to a
FREE non-alcoholic drink with a purchase of a Southern Style Buffet
or lunch or dinner entree at The Holiday Inn, I-295 and Commonwealth
Ave., Jacksonville, FL.
ALL WINNERS MUST PRESENT THEIR NUMBERED TICKET WHEN NOTIFIED!
THEREFORE, REMEMBER TO HOLD ONTO YOUR TICKET UNTIL AFTER THE
SHOPPING SPREE DRAWING!
PLEASE NOTE:
- If you have any ticket booklets sold by the end of October, please
contact Dick Keyworth at 272-6733 or Cell Phone 505-4627 to arrange for
turn-in.
- All ticket stubs, money and unsold tickets must be turned into Dick
Keyworth by Tuesday, November 13, 2007. Black Creek District
will be charged a 5% late fee for tickets turned in after the due date!
- Control all tickets - We'll be assessed $ .25 cents per ticket not
turned in.
